Brainrot Hospital is a first-person survival horror game that puts you in the role of a hospital employee working in a strange hospital where each patient needs to be examined before you decide whether or not to treat them. The initial job seems quite simple: admit patients, observe their condition, handle normal cases, and continue your shift. The problem is, some things in the hospital may not be what they seem.
The primary mechanism of Brainrot Hospital is observation. You need go around the hospital regions and get to know each patient and attentively see what's going on around them.
If the patient is completely normal, you can interact and treat them to continue your work. But when you detect an abnormality, the safe choice is not to admit that patient.
A notable point is that the game doesn't turn missions into pre-answered tests. Players must observe the environment themselves and remember any unusual changes. A tiny thing out of place can sometimes be a symptom that the situation has altered.

The suspense of Brainrot Hospital derives from the fact that you can’t fully trust what you’ve seen before. The hospital morphs in odd ways from shift to shift, making players have to look again rather than responding out of habit.
You also have to acquire brainrots while you work. However, racing around the hospital for stuff should not make you miss patients or worrisome symptoms.
But a checkup is not the goal. You want to strive to survive as many turns as possible. The more shifts you survive the better you rank on the leaderboard.
